Đặt vấn đề: Tỉ lệ sỏi ống mật chủ (SOMC) ở những trường hợp có sỏi túi mật chiếm khoảng 5-15%. Phẫu thuật cắt túi mật nội soi (CTMNS) sau nội soi mật tụy ngược dòng (ERCP) có tỉ lệ biến chứng và chuyển mổ hở cao hơn so với cắt túi mật thông thường do khả năng viêm dính không dự đoán trước được và thời điểm phẫu thuật thích hợp vẫn còn bàn cãi. Một số nghiên cứu gần đây khuyến cáo nên phẫu thuật cắt túi mật trong vòng 3 ngày sau ERCP.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả loạt ca, bao gồm 109 trường hợp được phẫu thuật cắt túi mật nội soi sau nội soi mật tụy ngược dòng trong thời gian từ tháng 1/2020 đến tháng 7/2023 tại Bệnh viện Hoàn Mỹ Sài Gòn.
Kết quả: Độ tuổi trung bình là 55, tỉ lệ nữ/nam là 1,5/1. Có 74 trường hợp được phẫu thuật trước 3 ngày, 15 trường hợp được phẫu thuật trước 7 ngày và 20 trường hợp sau 7 ngày. Thời gian trung bình từ khi nội soi mật tụy ngược dòng đến khi cắt túi mật 4,4 ngày. Viêm tụy cấp sau nội soi mật tụy ngược dòng 16 (14,7%). Thời gian phẫu thuật trung bình 77,6 phút, không có sự khác biệt về thời gian phẫu thuật giữa 3 nhóm (p=0.911). Không có trường hợp nào chuyển mổ hở. Không có biến chứng dò mật, tổn thương đường mật hay chuyển mổ hở. Không có sự khác biệt về mức độ dính trong mổ, thời gian nằm viện sau mổ giữa 3 nhóm. Thời gian nằm viện trung bình 3,1 ngày.
Kết luận: Phẫu thuật cắt túi mật nội soi có thể thực hiện sớm sau ERCP cho kết quả an toàn và nguy cơ biến chứng sau mổ thấp, giúp rút ngắn thời gian nằm viện.

The results of early laparoscopic cholecystectomy after endoscopic retrograde cholangiopancreatography for choledocholithiasis

Abstract
Introduction: The rate of common bile duct stones associated with gallstones accounts for about 5-15%. Laparoscopic cholecystectomy after Endoscopic Retrograde Cholangio-Pancreatography (ERCP) has a higher rate of complications and conversion to open surgery than conventional cholecystectomy due to the potential for unpredictable adhesions and the appropriate timing of surgery remains controversial. Several recent studies recommend cholecystectomy within 3 days of ERCP.
Patients and Methods: A descriptive case-series study, including 109 cases of laparoscopic cholecystectomy after ERCP from January 2020 to July 2023 at Hoan My Saigon Hospital.
Results: Mean age was 55, female/male ratio was 1.5/1. There were 74 cases of surgery before 3 days, 15 cases of surgery before 7 days and 20 cases after 7 days. The mean time from ERCP to laparoscopic cholecystectomy was 4.4 days. Acute pancreatitis after ERCP 16 (14.7%). Mean operation time was 77.6 minutes, there was no meaningful difference in operation time between 3 groups (p=0.911). There were no complications of biliary fistula, biliary tract injury or conversion to open surgery. There was no difference in the degree of intraoperative adhesions, postoperative hospital stay between the 3 groups. Average hospital stay was 3.1 days.
Conclusion: Laparoscopic cholecystectomy can be performed early after ERCP for safe results and low risk of postoperative complications in helping to shorten hospital stay.
