Đặt vấn đề: Đại tiện không tự chủ (ĐTKTC) làm giảm đáng kể chất lượng cuộc sống của người bệnh. Đo áp lực hậu môn trực tràng (ALHMTT) độ phân giải cao hỗ trợ chẩn đoán và điều trị ĐTKTC.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả chùm ca bệnh, hồi cứu trên các người bệnh ĐTKTC > 18 tuổi từ 01/2020 đến 08/2021.
Kết quả: 20 người bệnh với tỉ lệ nữ 45%, tuổi trung bình: 49,8 18,2. Kết quả đo ALHMTT giữa 2 nhóm ĐTKTC cấp kì (9/20) và thụ động (11/20) không có sự khác biệt trừ thể tích duy trì cảm giác muốn đại tiện và thể tích bơm bóng tối đa thấp hơn ở nhóm cấp kì (48,8 ± 11,3 so với 70,0 ± 21,8 ml và 90,0 ± 28,3 so với 127,5 ± 26,7 ml; p < 0,05). Áp lực trung bình của hậu môn thì nghỉ, nhíu, rặn, ho lần lượt là 48,4 mmHg, 99,6 mmHg, 60,3 mmHg và 63,1 mmHg. Ngưỡng nhận cảm của trực tràng và khả năng chịu đựng tối đa của trực tràng lần lượt là 27,5 ml và 112,5 ml. Có sự tương quan rõ ràng giữa sự giảm áp lực nhíu của hậu môn và ngưỡng nhận cảm của trực tràng với mức độ nặng của bệnh (r = -0,46, -0,49; p < 0,05).
Kết luận: Đo ALHMTT độ phân giải cao có giá trị trong chẩn đoán, đánh giá mức độ tổn thương, lên kế hoạch điều trị và tiên lượng với người bệnh ĐTKTC.

Resultats of high-resolution anorectal manometry (HRAM) for fecal incontinence in Viet Duc University Hospital during period 2020 – 2021

Introduction: Fecal incontinence significantly reduces the patients quality of life. High resolution anorectal manometry is a new effective way for fecal incontinences assessment and treatment planning.
Patients and methods: A case series study of 20 patients diagnosed with fecal incontinence (FI) in Viet Duc University Hospital.
Results: 20 patients with FI include 9 women (45%), 11 men (55%), mean age: 49.8. There was no difference between the two groups of urge FI (9/20) and passive FI (11/20) except the balloon volume that give constant sensation to defecate and the max tolerable volume which was lower in the urge FI group (48.8 ± 11.3 vs 70.0 ± 21.8 ml and 90.0 ± 28.3 vs 127.5 ± 26.7 ml; p < 0.05). The mean anal pressure at rest, squeezing, straining, and coughing was 48.4 mmHg, 99.6 mmHg, 60.3 mmHg and 63.1 mmHg, respectively. The rectal first sensation threshold and max tolerance volume were 27.5 ml and 112.5 ml, respectively. Maximum anal squeeze pressure, first sensation volume decreased significantly with increasing FI severity (r = -0.46, -0.49, p < 0.05).
Conclusion: High-resolution anorectal manometry is valuable in diagnosis, assessment, treatment and prognosis in patients with fecal incontinence.
