Đặt vấn đề: Phẫu thuật Robot cắt tuyến tiền liệt tận gốc là một tiến bộ vượt bậc hiện nay với nhiều tính năng ưu việt. Tuy nhiên đối với nhóm ung thư tuyến tiền liệt nguy cơ cao, đặc biệt là tiến triển tại chỗ, đây vẫn là một thách thức. Hiện nay phương pháp này cũng gặp phải một số khó khăn nhất định đối với ung thư ở giai đoạn tiến triển tại chỗ hoặc nguy cơ và rất cao. Chúng tôi so sánh kết quả của phương pháp cắt tuyến tiền liệt tận gốc với sự hỗ trợ Robot với mổ mở trên những người bệnh ung thư giai đoạn cao và rất cao.
Đối tượng và phương pháp nghiên cứu: Tất cả những trường hợp ung thư tuyến tiền liệt nguy cơ cao và rất cao được phẫu thuật Robot và phẫu thuật mở cắt tuyến tiền liệt tận gốc tại bệnh viện Chợ Rẫy từ 01/2011 đến 12/2022. Ung thư TTL nguy cơ cao được thỏa một trong những tiêu chuẩn sau: PSA > 20 ng/ml, giai đoạn T3a – T4, Gleason score theo ISUP 4 – 5. Các biến số dịch tễ học, lâm sàng, cận lâm sàng, kết quả phẫu thuật, biến chứng được ghi nhận. Đánh giá thời gian sống còn chung, sống còn không tiến triển, sống còn không tái phát sinh hóa dựa trên phân tích Kaplan-Meier.
Kết quả: Tổng cộng 47 người bệnh ung thư tuyến tiền liệt nguy cơ cao được phẫu thuật cắt TTL tận gốc tại bệnh viện Chợ Rẫy, trong đó có 34 TH phẫu thuật Robot và 13 TH phẫu thuật mở. Không có sự khác nhau về độ tuổi trung bình, BMI, giá trị PSA, thể tích tuyến tiền liệt, giai đoạn bệnh và độ ác tính của ung thư. Phẫu thuật Robot có thời gian mổ dài hơn nhưng có lượng máu mất ít hơn, tỉ lệ truyền máu ít hơn so với mổ mở. Khả năng tiểu kiểm soát cao hơn trong nhóm phẫu thuật Robot. Tỉ lệ biên phẫu thuật dương tính, PSA tăng dai dẳng và tái phát sinh hóa tương đương nhau ở cả 2 phương pháp. Thời gian sống còn không tái phát sinh hóa không khác nhau giữa 2 nhóm, tuy nhiên thời gian sống không di căn và sống còn chung cao hơn ở nhóm phẫu thuật Robot so với những người bệnh được mổ mở.
Kết luận: Phẫu thuật Robot cắt tuyến tiền liệt tận gốc mang lại hiệu quả và an toàn về mặt biến chứng và kết cục ung thư học tốt hơn so với phẫu thuật mở trên nhóm người bệnh ung thư tuyến tiền liệt nguy cơ cao, đặc biệt là đã tiến triển tại chỗ. Mặc dù vậy, phẫu thuật Robot nói riêng và các kỹ thuật cắt TTL tận gốc khác cũng nên được xem là phương pháp điều trị đầu tay trong liệu pháp điều trị đa mô thức đối với nhóm người bệnh này.

The comparative study of outcomes between Robot-assisted and open radical prostatectomy in high-risk prostate cancer

Abstract
Introduction: Radical prostatectomy is the standard treatment for patients with localized prostate cancer (PCa). Nevertheless, high-risk PCa are considered challenging in surgical treatment even with Robot system, which is the latest innovation in surgical technique. We report the short-term and long-term outcomes of Robotic radical prostatectomy versus open procedure in patients with high-risk prostate cancer.
Patients and methods: Patients with high-risk prostate cancer underwent radical prostatectomy at Cho Ray hospital between 2011 and 2022. High-risk prostate cancer is defined as patients with PSA > 20 ng/ml or cT3-4 or Gleason score ISUP group 4 – 5. Demographic, clinical, sub-clinical parameters, and results of surgery were recorded. Kaplan-Meier survival analysis was used to calculate Biochemical progression-free survival (BPFS), Metastasis-free survival (MFS) and Overall survival (OS) rate.
Results: A total of 47 patients with high-risk PCa were treated surgically by radical prostatectomy including 34 cases of robotic and 13 cases of open procedures. There were no statistically differences between two group in terms of age, BMI, PSA value, prostate volume and cancer stages and grades. Robotic prostatectomy was associated with longer OR time, less blood loss and lower transfusion rate. Urinary continence function was higher in Robotic group. Positive margin, persistent PSA and biochemical recurrence were comparable between Robotic and open group. Patients underwent Robotic prostatectomy have increased survival in terms of Metastasis-free survival (MFS) and Overall survival (OS).
Conclusion: Robotic radical prostatectomy, which brings beneficial effects and safety regarding not only functional but also oncological outcomes compares with open surgery. Nevertheless, it has been considered as a primary step in multimodality treatment in patients with high-risk prostate cancer.
