Abstract
Introduction: Surgery is the mainstay of treatment for pancreatic head cancer for improvement of patient survival. Studies in Vietnam have focused less on survival after surgery. Therefore, we conducted a study to evaluate the results of surgical treatment of pancreatic head cancer at Viet Duc Hospital.
Patients and methods: A Retrospective study of patients with pancreatic head cancer have been operated at Viet Duc University Hospital from 01/2014 to 12/2015.
Results: There were 83 pancreatic head cancer patients operated at Viet Duc hospital. The mean age was 57.7 ± 11.5 (age), male and female were 57.8% and 42.2%, respectively. 18.1% of patients had malnutrition before surgery; The rate of tumor resection (Whipple procedure) was 57.9%. The rate of bypass was 36.1%, the rate of exploratory surgery was 6%. The postoperative complications were bleeding, pancreatic leak, Choledochojejunostomy leak accounted for 6%; 3.6% and 1.2%, respectively. The postoperative mortality rate was 2.4%. The mean overall survival time was 12.35 ± 1.36 months; survival rate after 2 years is 20.3%. Factors such as resection of pancreatic head tumor, no lymph node metastasis, and no distant metastasis all increased the patient’s survival time (p<0.05)
Conclusion: Surgical treatment of pancreatic head cancer is feasible and safe; and pancreaticoduodenectomy increase the patient’s survival time

Đặt vấn đề: Phẫu thuật là phương pháp điều trị chính của ung thư đầu tụy, giúp cải thiện thời gian sống thêm của người bệnh. Các nghiên cứu ở Việt Nam ít tập trung vào thời gian sống thêm sau mổ. Do đó chúng tôi tiên hành nghiên cứu đánh giá kết quả điều trị ung thư đầu tụy bằng phẫu thuật tại bệnh viện hữu nghị Việt Đức.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu hồi cứu, các người bệnh ung thư đầu tụy được phẫu thuật tại bệnh viện hữu nghị Việt Đức từ tháng 1/2014 đến tháng 12/2015.
Kết quả: Từ T1/2014 đến tháng 12/2015, có 83 người bệnh ung thư đầu tụy được phẫu thuật tại bệnh viện Việt Đức. Tuổi trung bình là 57,7 ± 11,5 (tuổi), tỷ lệ nam, nữ lần lượt là 57,8% và 42,2%. Có 18,1% người bệnh có suy dinh dưỡng trước mổ; Tỷ lệ cắt được u (cắt khối tá tụy) là 57,9%. Tỷ lệ nối tắt là 36,1%, tỷ lệ mổ thăm dò là 6%. Các biến chứng chảy máu sau mổ, rò tụy, rò miệng nối mật-ruột lần lượt là 6%; 3,6% và 1,2%. Tỷ lệ tử vong sau mổ là 2,4%. Thời gian sống thêm trung bình toàn bộ là 12,35 ± 1,36 tháng; tỷ lệ sống thêm sau 2 năm là 20,3%. Các yếu tố như cắt được khối tá tụy, không có di căn hạch, không có di căn xa đều làm tăng thời gian sống thêm của người bệnh(p<0,05)
Kết luận: Phẫu thuật điều trị ung thư đầu tụy nói chung và phẫu thuật cắt khối tá tụy nói riêng là khả thi, an toàn; và phẫu thuật cắt khối tá tụy làm tăng thời gian sống thêm của người bệnh
