Đặt vấn đề: Gây mê toàn thể và phẫu thuật nội soi ổ bụng làm chức năng hô hấp suy giảm do giảm dung tích cặn chức năng, xẹp phổi và thậm chí là tổn thương phổi liên quan đến thở máy[1-3]. Các chiến lược thông khí nhằm bảo vệ phổi và cải thiện oxy hóa máu trong phẫu thuật đã được nghiên cứu trong đó có nghiệm pháp huy động phế nang. Tuy nhiên, kết quả các nghiên cứu còn chưa thống nhất, vì vậy chúng tôi thực hiện đề tài này với câu hỏi nghiên cứu: “Nghiệm pháp huy động phế nang trong gây mê phẫu thuật nội soi ổ bụng có tác dụng như thế nào lên tình trạng oxy hóa máu?”
Đối tượng và phương pháp nghiên cứu: Thử nghiệm lâm sàng có đối chứng ngẫu nhiên trên 61 người bệnh được chia thành hai nhóm: nhóm chứng gồm 30 người bệnh được thông khí trong mổ bằng phương thức kiểm soát thể tích (VCV), VT 8 ml/kg cân nặng lý tưởng (IBW), PEEP 5 cmH2O và không HĐPN trong mổ; nhóm can thiệp (nhóm huy động phế nang) gồm 31 người bệnh được thông khí trong mổ bằng phương thức VCV, VT 8ml/kg IBW, PEEP 5 cmH2O và có HĐPN trong mổ.
Kết quả: Thời điểm kết thúc phẫu thuật, PaO2 của nhóm HĐPN là 258,95 ± 47.94 mmHg cao hơn so với nhóm chứng là 228,93 ± 45,67 mmHg
(p < 0,05). Tỷ số PaO2/FiO2 của nhóm HĐPN là 517,90 ± 95,87 mmHg cao hơn so với nhóm chứng là 457,86 ± 91,33 mmHg (p < 0,05). Sau khi người bệnh được rút ống nội khí quản 60 phút và thở khí trời trong 15 phút, PaO2 của nhóm HĐPN là 100,87 ± 7,27 mmHg cao hơn so với nhóm chứng là 88,89 ± 3,53 mmHg với p < 0,05; tỷ số PaO2/FiO2 của nhóm HĐPN là 480,35 ± 34,64 mmHg cao hơn so với nhóm chứng là 417,30 ± 16,86 mmHg (p < 0,05); SpO2 của nhóm HĐPN là 98,22 ± 1,31% cao hơn so với nhóm chứng là 97,10 ± 1,88% (p < 0,05).
Kết luận: Nghiệm pháp huy động phế nang giúp tăng oxy hóa máu trong quá trình gây mê cho phẫu thuật nội soi ổ bụng.

Abstract
Introduction: General anesthesia and laparoscopic surgery impair respiratory function followed by reduced functional residual capacity, atelectasis, and even ventilator-induced lung injury. Ventilation strategies protecting the lungs and improving oxygenation during surgery have been studied, including lung recruitment maneuvers. However, the results of studies are not consistent, so we carry out this study with the research question: “How effectively does lung recruitment maneuver impact on blood oxygenation status in laparoscopic surgery?”
Patients and methods: a RCT study on 61 patients divided into two groups: control group (30 patients) undergoing intraoperative ventilation with volume control ventilation (VCV) mode, volume tidal (VT) 8 ml/kg ideal body weight (IBW), PEEP 5 cmH2O with no lung recruitment maneuver (LRM) and intervention group (31 patients) undergoing intraoperative ventilation with VCV mode, VT 8 ml/kg IBW, PEEP 5 cmH2O, with lung recruitment maneuver (LRM).
Results: At the end of surgery, PaO2 in the LRM group (258.95 ± 47.94 mmHg) was higher than that in the control group (228.93 ± 45.67 mmHg), p < 0.05; the PaO2/FiO2 value in the LRM group (517.90 ± 95.87 mmHg) was higher than that in the control group (457.86 ± 91.33 mmHg), p < 0.05. In the recovery room, after being extubated in 60 minutes and spontaneous breathing in 15 minutes, the PaO2 value in the LRM group (100.87 ± 7.27 mmHg) was higher than that in the control group (88.89 ± 3. 53 mmHg), p < 0.05; the PaO2/FiO2 value in the LRM group (480.35 ± 34.64 mmHg) was higher than that in the control group (417.30 ± 16.86 mmHg), p < 0.05; SpO2 in the LRM group (98.22 ± 1.31%) was higher than that in the control group (97.10 ± 1.88%), p < 0.05.
Conclusion: Lung recruitment maneuver increases oxygenation in laparoscopic surgery.
