Đặt vấn đề: Phẫu thuật nội soi một lỗ điều trị u trung thất mới được áp dụng rộng rãi trong thời gian gần đây. Tính hiệu quả và khả thi của phẫu thuật này trong điều kiện Việt Nam vẫn đang được nghiên cứu đánh giá. Nghiên cứu này nhằm báo cáo kết quả ngắn và trung hạn của phẫu thuật nội soi một lỗ điều trị các khối u trung thất trước tại khoa Phẫu thuật lồng ngực – Bệnh viện Bạch Mai.
Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả tiến cứu các người bệnh bị u (hay nang) trung thất trước, điều trị bằng phẫu thuật nội soi một lỗ từ tháng 1 năm 2017 đến tháng 12 năm 2020. Phẫu thuật nội soi một lỗ được thực hiện chỉ qua một đường mở ngực nhỏ khoảng 3 cm, ở vị trí khoang liên sườn 4 hoặc 5 đường nách giữa; ống kính nội soi 10 mm x 300 và các dụng cụ phẫu thuật được sử dụng đồng thời qua lỗ mở nhỏ này trong suốt quá trình phẫu thuật.
Kết quả: Bao gồm 52 người bệnh, với 31 nữ giới (59,6%), tuổi trung bình 51 ± 15 tuổi. Giải phẫu bệnh nhiều nhất là u tuyến ức (33), rồi đến u quái (11), u thần kinh (1), nang màng tim-màng phổi (2), nang phế quản (1), u lympho (1), quá sản tuyến ức (3). Kích thước trung bình của u trên chụp cắt lớp vi tính là 5,4 ± 2,3 cm. Thời gian mổ trung bình là 93,4 ± 43,4 phút. Chiều dài vết mổ trung bình là 2,9 ± 0,8 cm. Thời gian rút dẫn lưu trung bình 3,5±1,3 ngày; thời gian nằm viện sau mổ trung bình 7,2 ± 5,5 ngày. Phẫu thuật kiểu nội soi một lỗ hoàn toàn có 46 ca (88,5%), nội soi hỗ trợ có 3 ca (5,7 %), 3 ca cần thêm 1 lỗ trocar (5,7 %). Mức độ đau sau mổ nhẹ-vừa chiếm 82,7%. Biến chứng sau mổ gồm: chậm liền vết mổ, tràn dưỡng chấp màng phổi, xẹp phổi, tan máu, ổ cặn màng phổi – mỗi loại có 1 ca (1,9%), và 4 ca liệt hoành (7,7%). Không có tử vong trong và sau mổ. Theo dõi sau 12 tháng không có trường hợp nào bị u tái phát.
Kết luận: Phẫu thuật nội soi một lỗ cho phép điều trị một số thể u trung thất trước có kích thước dưới 6 cm, theo dõi ngắn và trung hạn cho kết quả tốt.

Abtracts
Introduction: Single-port video assisted thoracic surgery (VATS) for mediastinal tumors has been widely applied in recent years. The effectiveness and feasibility of this procedure in Vietnamese conditions is still being studied and evaluated. This study aims to report the short and medium-term results of single-port VATS for anterior mediastinal tumors at the department of Thoracic Surgery – Bach Mai Hospital.
Patients and methods: Prospective descriptive study of patients with anterior mediastinal tumors (or cysts), treated with single-port VATS from January 2017 to December 2020. Single-port VATS performed only through a small thoracotomy about 3 cm, in the 4th or 5th intercostal space in the middle axillary line; optic 10 mm x 300 and surgical instruments are used simultaneously through this small opening during surgery.
Results: Including 52 patients, with 31 female (59.6%), mean age 51 ± 15 years. The most common pathology is thymoma (33), then teratoma (11), neuroma (1), pericardial-pleural cyst (2), bronchial cyst (1), lymphoma (1), thymic hyperplasia (3). The average size of tumor on computed tomography was 5.4 ± 2.3 cm. The average surgery time was 93.4 ± 43.4 minutes. The average incision length was 2.9 ± 0.8 cm. The average draining time is 3.5 ± 1.3 days; The average postoperative hospital stay was 7.2 ± 5.5 days. Complete laparoscopic surgery with a single-port VATS was done in 46 cases (88.5%), laparoscopic support in 3 cases (5.7 %), 3 cases required an additional trocar (5.7%). The degree of postoperative pain was mild-moderate 82.7%. Postoperative complications include: wound infection, chylous pleural effusion, atelectasis, hemolysis, empyema – 1 case for each (1.9%), and 4 cases of diaphragmatic paralysis (7.7%). There was no intraoperative and postoperative mortality. Follow-up after 12 months, there was no case of tumor recurrence.
Conclusion: Single-port VATS allows the treatment of some anterior mediastinal tumors less than 6 cm in size, with good results in short and medium-term follow-up.
