Đặt vấn đề: Ung thư đại trực tràng (UTĐTT) là loại ung thư đứng hàng thứ 3 trong các loại ung thư ở Việt Nam. Trước đây, di căn xa do UTĐTT thường chỉ được điều trị triệu chứng, và tỉ lệ sống còn của người bệnh chỉ khoảng 5% sau 5 năm. Gần đây, phẫu thuật cắt gan di căn với mục đích triệt để đã cho kết quả rất đáng khích lệ với tỉ lệ sống còn sau 5 năm lên đến > 50%. Hiện nay, chưa có khuyến cáo cụ thể cho chọn lựa cắt ung thư đại trực tràng đồng thời với cắt gan hay cắt gan sau cắt ung thư đại trực tràng. Tuy nhiên một số nghiên cứu gần đây đã cho thấy cắt đồng thời ung thư đại trực tràng kèm di căn gan cho kết quả tương đương với phẫu thuật nhiều thì, mặt khác có hiệu quả về kinh tế và điều trị cho người bệnh. Ở Việt Nam, chúng ta chưa có nhiều công trình nghiên cứu về lĩnh vực này.
Phương pháp: Hồi cứu mô tả loạt ca. Các trường hợp ung thư đại trực tràng có di căn gan đồng thời được thực hiện cắt đồng thời ung thư đại trực tràng kèm di căn gan được đưa vào nghiên cứu. Chúng tôi đánh giá kết quả phẫu thuật, tỉ lệ tai biến, biến chứng sau mổ và các kết quả về mặt ung thư học trong thời gian theo dõi.
Kết quả: Từ 5/2020 đến 5/2021, chúng tôi đã thực hiện 22 trường hợp cắt đồng thời ung thư đại trực tràng kèm di căn gan. Tỉ lệ phẫu thuật nội soi là 76,1%. Có 14 trường hợp được hoá trị trước mổ (63,6%). Có 3 trường hợp di căn gan được đánh giá không cắt được đã được phẫu thuật cắt gan sau hoá trị tân hỗ trợ. Tỉ lệ cắt gan > 2 u đạt 33,3%. Chúng tôi ghi nhận 1 trường hợp viêm phổi nặng sau mổ cắt gan đa ổ, và người bệnh tử vong tại ICU sau 5 ngày. Tỉ lệ tử vong 30 ngày sau mổ là 4,5%. Các trường hợp còn lại không ghi nhận tai biến hay biến chứng sau mổ. Thời gian theo dõi trung bình 35,5 tháng (2 – 120 tháng), chúng tôi ghi nhận 5 trường hợp (23,8%) tái phát di căn gan. Di căn phổi gặp 2 ca (9,5%). Không ghi nhận tái phát tại chỗ của đại trực tràng. Tỉ lệ sống còn chung và sống còn không bệnh sau 5 năm lần lượt là 45,9% và 29,0%.
Kết luận: Phẫu thuật cắt đồng thời ung thư đại trực tràng kèm di căn gan là phẫu thuật khả thi và an toàn với các kết quả phẫu thuật và ung thư học chấp nhận được. Phương pháp phẫu thuật nội soi cắt đại trực tràng phối hợp mổ mở cắt gan có thể là phương pháp tiếp cận phù hợp với những trung tâm còn hạn chế về phương tiện và nguồn lực cho phẫu thuật cắt gan nội soi. Cần phối hợp tốt hơn trong hội chẩn đa mô thức để tối ưu hoá kết quả phẫu thuật của người bệnh.

Long-term results of simultaneous resection of colorectal liver metastasis

Abstract
Colorectal cancer (CRC) is the third leading type of cancer in Vietnam. In the past, distant metastases from colorectal cancer were often treated symptomatically, and the patient survival rate was only about 5% after 5 years. Recently, radical resection of metastatic liver disease has shown very encouraging results with 5-year survival rates up to > 50%. Until now, there was no consensus on simultaneous resection of colorectal liver metastasis. However, recent publications had shown comparable results between simultaneous resection of colorectal liver metastasis and staged liver resection, meanwhile, simultaneous resection offers benefits to patients and the health care system. In Vietnam, we do not have many studies in this field.
Methods: This is a retrospective case series study. Simultaneous resection of colorectal liver metastasis were included into the study. We evaluated surgical outcomes, complication rates, postoperative complications and oncological outcomes during follow-up.
Results: From 5/2020 to 5/2021, we performed 22 cases of simultaneous resection of colorectal liver metastasis. In which, laparoscopic surgery was 76.1%. There were 14 cases (63.6%) receiving chemotherapy before surgery. Three cases of nonresectable metastatic liver lesions has become resectable after neoadjuvant chemotherapy.
The rate of liver resection > 2 tumors reached 33.3%. We noted 1 case of severe postoperative pneumonia, and the patient died in the ICU after 5 days. There was no other postoperative complication. The 30-day mortality rate after surgery was 4,5%. The average follow-up time was 35.5 months (2 – 120 months), we noted 5 cases (23.8%) with recurrence of liver metastases. Lung metastasis occurred in 2 cases (9.5%). No colorectal recurrence was noted. Five year overall survival and disease-free survival were 45.9% and 29,0%, respectively.
Conclusion: Simultaneous resection of colorectal liver metastasis is feasible and safe with acceptable surgical and oncological outcomes. Laparoscopic colorectal resection in combined with open liver resection could be an appropriate choice for medical centers with limited resources for laparoscopic liver surgery. It is important to coordinate well in multidisciplinary management in order to optimize patient’s surgical outcome.
