Đặt vấn đề: Gãy kín thân xương đùi thường được điều trị bằng phẫu thuật đóng đinh nội tủy. Trong nghiên cứu này, chúng tôi báo cáo một kỹ thuật đóng đinh nội tủy xuôi dòng điều trị gãy kín thân xương đùi qua một đường mở nhỏ.
Đối tượng và phương pháp nghiên cứu: Từ tháng 10/2020 – 5/2021, kỹ thuật này được sử dụng trên 30 người bệnh gãy kín thân xương đùi. Người bệnh đã được theo dõi sau phẫu thuật để đánh giá lâm sàng và trên phim X quang sau mổ 3 tháng, 6 tháng.
Kết quả: Có 30 người bệnh được đánh giá sau mổ 3 tháng, và 21 người bệnh được đánh giá sau mổ 6 tháng. Sau mổ không có người bệnh nào nhiễm trùng hay cần phẫu thuật lại. Trong 21 người bệnh sau mổ 6 tháng, có 19 bênh nhân (90,5%) đã liền xương hoàn toàn, đánh giá kết quả chức năng theo Klemm và Borner có 85,7% (18/21) người bệnh rất tốt, 9,5% (2/21) người bệnh có kết quả tốt, 4,8% (1/21) người bệnh kết quả trung bình, không có người bệnh kết quả kém.
Kết luận: Phẫu thuật đóng đinh nội tủy xuôi dòng điều trị gãy kín thân xương đùi qua một đường mở nhỏ cho thấy nhiều ưu điểm: bảo tồn phần mềm, màng xương, bảo tồn máu tụ ổ gãy, không cần máy C-arm, không cần bàn chỉnh hình, rút ngắn thời gian cuộc mổ, thích hợp với người bệnh đa chấn thương, người bệnh béo phì, gãy 1/3 dưới thân xương.

Antegrade intramedulary nailing result of closed femoral shaft fracture through a small incision

Abstract
Introduction: Closed femoral shaft fractures are usually treated with intramedullary nailing. In this study, we report a surgical technique: Antegrade intramedullary nailing of closed femoral shaft fracture through a small incision.
Patients and Methods: From October 2020 to May 2021, this technique was used on 30 patients with closed femoral shaft fractures. The patient was followed up after surgery for clinical evaluation and X ray at 3 months and 6 months.
Results: 30 patients were followed up in the 3 months and 21 patients were followed up in 6 months. All 30 patients did not have any infection, and none of the patients required reoperation. 19 fractures (90,5%) healed in the first 6 months. Evaluation of functional outcomes according to Klemm and Borner. After 6 months, 85.7% (18/21) patients had very good results, 9,5% (2/21) patients had good results and 4,8% (1/21) average results, no patients had poor results.
Conclusion: The advantages of antegrade intramedullary nailing of closed femoral shaft fracture through a small incision are preservation of soft tissue, preservation of broken hematoma, preservation of periosteum, no need for C-arm machine, no need for fracture table, shortened operation time, suitable for multiple trauma, obese patients, distal shaft fractures.
