Đặt vấn đề: Đánh giá kết quả phẫu thuật bắc cầu chủ vành kinh điển điều trị bệnh cơ tim thiếu máu cục bộ ổn định trên các người bệnh có phân suất tổng máu (Ejection Fraction -EF) giảm < 50%).
Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả hồi cứu, theo dõi dọc các người bệnh mổ bắc cầu chủ vành phân suất tống máu giảm có sử dụng tuần hoàn ngoài cơ thể và ngừng tim tại Trung tâm Tim mạch và Lồng ngực Bệnh viện Hữu nghị Việt Đức từ tháng 09/2019 đến tháng 09/2022. Phân tích, xử lý số liệu bằng phần mềm SPSS 20.0.
Kết quả: Có 20 người bệnh đủ điều kiện tham gia nhiên cứu, Nam giới chiếm 75% (15) người bệnh. Nhóm EF giảm nhẹ (EF từ 41 – 49%) có 11(55%) người bệnh với tuổi trung bình là 70 ± 7,65 tuổi và nhóm EF giảm nhiều (EF ≤ 40%) có 9 (45%) người bệnh với tuổi trung bình là 64 ± 7,68 tuổi. Tỷ lệ hở van hai lá vừa đến nhiều chiếm 70% cả hai nhóm và đặc biệt cao ở nhóm EF giảm nhiều gặp 100% người bệnh. 77,78% người bệnh có phẫu thuật kèm theo. Không có sự khác biệt về biến cố trước và trong phẫu thuật giữa nhóm EF giảm nhẹ và EF giảm nhiều. EF được cải thiện có ý nghĩa thống kê ở cả hai nhóm, nhóm có EF giảm nhẹ từ 45 ± 2,73% lên mức 50 ± 11,49% (Tăng 5% và P= 0,04) và ở nhóm EF giảm nhiều là từ 35 ± 2,23% lên mức 43 ± 13,57% (Tăng 8% và P= 0,03). Có 2 trường hợp tử vong chiếm 10%.
Kết luận: Phẫu thuật bắc cầu chủ vành ở người bệnh có EF thấp tại bệnh viện Hữu Nghị Việt Đức giai đoạn 2019-2022 là an toàn và cho kết quản sớm tốt, phân suất tống máu sau mổ từng bước hồi phục.

Outcomes of classic coronary artery bypass grafting in selective patients with reduced ejection fraction at Viet Duc University Hospital

Abstract
Introductions: To evaluate the results of classic coronary artery bypass grafting to treat ischemic heart disease in selective patients with reduced ejection fraction (EF) <50%.
Patients and method: This was a retrospective, descriptive study of selective patients with EF who underwent classic aortic-coronary bypass grafts at the cardiovascular and thoracic center of Viet Duc University Hospital from September 2019 to September 2022. Data was collected and analyzed by SPSS 20.0 software.
Results: There were 20 patients included in the study: men accounted for 75% (15) of the patients. The mildly reduced EF group (EF from 41 to 49%) was 11 (55%) patients with an average age of 70 ± 7.65 years, and the severely reduced EF group (EF ≤ 40%) was 9 (45%) patients, of them an average age was 64 ± 7.68 years. The rate of moderate to severe mitral valve regurgitation accounted for 70% in both groups and was especially high in the severely reduced EF group, accounting for 100% of patients. 77.78% of patients had concomitant procedures. There was no difference in preoperative and intraoperative incident between the mildly reduced EF and greatly reduced EF groups. EF was improved statistically significant for both groups; the group with EF decreased slightly from 45 ± 2.73% to 50 ± 11.49% (a 5% increase and P = 0.04), and in the group with EF decreased significantly from 35 ± 2.23% to 43 ± 13.57% (an increase of 8% and P = 0.03). The mortality in hospital observed in 10%.
Conclusion: Classic coronary artery bypass grafting for the selective patients with EF <50% at Viet Duc University Hospital during the period 2019–2022 was safe, with good early results. EF was significantly improved in both groups.
