Kết quả phẫu thuật nội soi điều trị viêm ruột thừa ở phụ nữ có thai tại Bệnh viện Bạch Mai

Đặt vấn đề: Viêm ruột thừa cấp ở phụ nữ mang thai có sự khác biệt so với người bình thường nên chẩn đoán, điều trị có những khó khăn nhất định. Nghiên cứu nhằm mô tả đặc điểm lâm sàng, cận lâm sàng và kết quả phẫu thuật nội soi điều trị viêm ruột thừa cấp ở phụ nữ có thai.

Đối tượng và phương pháp: Hồi cứu các ca bệnh tại Bệnh viện Bạch Mai từ 1/2020 đến 12/2022, thu thập các dữ liệu lâm sàng, cận lâm sàng và các kết quả chính của phẫu thuật.

Kết quả: Tổng số 57 người bệnh, tuổi trung bình 27,7 (19 – 38) tuổi, tỷ lệ nhóm tuổi ≤ 30, 31 – 35 và > 35 tuổi lần lượt là 70,1%, 24,6% và 5,3%. Tỷ lệ sản phụ có tuổi thai 3 tháng đầu, 3 tháng giữa và 3 tháng cuối là 26,3%, 59,6% và 14,1%. Diễn biến bệnh ≤ 24 giờ và > 24 giờ là 63,1% và 36,9%. Dấu hiệu đau bụng cấp, sốt và nôn là 100%, 59,7% và 35,1%. Tỷ lệ bạch cầu tăng là 96,5%. Siêu âm thấy ruột thừa to và thâm nhiễm mỡ là 98,3% và 100%. Ruột thừa viêm ở đầu, thân và vỡ là 42,1% và 58,9% và 33,3%. Thời gian mổ và nằm viện là 56,7 (40 – 120) phút và 3,1 (1-7) ngày. Đặt trocar kiểu A hoặc B áp dụng chủ yếu cho thai kỳ đầu và giữa 84,21%. Biến chứng gồm 1 ca áp xe tồn dư và 3 ca dọa sảy thai.

Kết luận: viêm ruột thừa có thể gặp ở cả ba kỳ của quá trình thai nghén, có trường hợp ruột thừa đã vỡ nhưng phẫu thuật nội soi vẫn là phương pháp điều trị khả thi và hiệu quả.

Outcomes of laparoscopic appendectomy for pregnant at Bach Mai Hospital

Abstract

Introduction: Acute appendicitis in pregnant women is different from that in normal people. Therefore, treatment has certain difficulties. The study aims to describe the clinical characteristics and results of laparoscopic appendectomy.

Patients and Methods: The clinical data of all patients who underwent laparoscopic appendicectomy during pregnancy at Bachmai Hospital between January 2020 and December 2022.

Results: Fifty-seven patients underwent laparoscopic appendicectomy during pregnancy. The mean age was 27.7 (19 – 38) years. The rate of the age groups ≤ 30, 31 – 35, and > 35 years was 70.1%, 24.6%, and 5.3%, respectively. The percentage of the first, second, and third trimesters was 26.3%, 59.6%, and 14.1%. The disease progression ≤ 24 hours and 24 hours > was 63.1% and 36.9%. Signs of abdominal pain, fever, and vomiting were 100%, 59.7%, and 35.1%, respectively. Abdominal ultrasound showed that the enlarged appendix and fat infiltrated at 98.3% and 100%. Inflammatory appendicitis in the head, body, and rupture was 42.1%, 58.9%, and 33.3%. The mean operation time and hospital length stay were 56.7 (40 – 120) minutes and 3.1 (1-7) days. Insertion of trocar type A or B was mainly applied to early and middle pregnancy period in 84.21%. Complications included one intra-abdominal abscess and miscarriages were observed.

Conclusions: Appendicitis can be seen in all three trimesters of pregnancy. Laparoscopic appendectomy can be safely and effectively performed for all periods of pregnancy and in complicated case such as an appendix ruptures..
